Folic acid — the synthetic form of folate used in most supplements and fortified foods — must be converted by an enzyme called MTHFR before your body can use it. An estimated 30–40% of people carry variants of the MTHFR gene (C677T) that reduce this enzyme's efficiency, with homozygous carriers experiencing up to a 70% reduction in conversion capacity. For these individuals, taking folic acid means meeting the label requirement while potentially remaining functionally insufficient at the cellular level.
